The 2nd Annual “Poppy Foundation” Golf Tournament

There are (2) ways to participate in this tournament!
1) Become a sponsor and enjoy the prestige and visibility of supporting a worthy cause.
All sponsors are welcome to send business cards or advertising flyers to be
inserted in each golfers tee bag.
$100 Gift Bag Sponsors company name will be on our thank you display sign
$150 Tee Box Sponsors includes your company name on sign at designated hole
$500 Trophy Sponsor includes 2 guest to attend event for golf, banquet and signage on designated hole
OR you can help by providing a raffle prize or donation for our awards luncheon.
2) Participate as a team or individual in the tournament. Team donations (4) players are $300 or $75 per player. In addition to a terrific outing, you’ll be eligible for great raffle prizes and contests. $450 includes Team and a Tee Sponsorship.
